About the Project

HealthHIV serves as the National Center for HIV Care in Minority Communities (NCHCMC), the HIV/AIDS Bureau's major effort under the auspices of the national AETC program umbrella, to reach beyond the Ryan White-funded community and address the evolving needs and nature of HIV care and treatment within highly impacted communities of color.  The purpose of this project is to develop, improve, and enhance the organizational capacity of non-Ryan White funded community health centers (CHCs) to provide primary medical care and treatment to racial and ethnic minorities living with or affected by HIV/AIDS.

Led by HealthHIV, in collaboration with the National Association of Community Health Centers (NACHC), the NCHCMC will support the President's National HIV/AIDS Strategy in reducing HIV related health disparities by expanding HIV care and treatment capacity at the community level and facilitating linkages. The NCHCMC also sets out to increase the number of people who know their status by building on federal and health center HIV testing and screening capacity-building efforts.

Target Organizations

Organizations whose primary service delivery site is physically located in or near a community whose residents are at risk for or living with HIV and serve predominantly racial/ethnic minorities.  Preference will also be given to organizations located in geographic areas with a high burden of HIV disease with relatively low resources.   

This opportunity is open to organizations meeting the following three eligibility criteria:

Federally Qualified Health Centers (FQHC) or FQHC Look-Alikes
No direct funding from the Ryan White HIV/AIDS Program (sub-contractors and indirectly funded organizations are eligible)
A minimum of 4 years providing primary medical care to racial/ethnic minority communities
Through this project, we are looking for strong health centers to demonstrate successful integration of HIV care and treatment into primary care services. While not required, the following characteristics of health centers which may be more successful in this project will be considered:

• Demonstrated commitment and intent to deliver HIV care and treatment services
• Senior leadership (CEO, CFO, CMO particularly) in place for three years or more
• Accreditation from Joint Commission or AAAHC
• Established quality improvement program and responsible staff person(s)
• Capacity to manage patients through electronic health records, practice management systems or registries
• Electronic health records or patient management systems linked to revenue management systems
• Ability to collect and report patient level HIV data electronically

This project is funded by the Department of Health and Human Services (DHHS), Health Resources and Services Administration (HRSA) HIV/AIDS Bureau (HAB) through the AETC Program and through the DHHS Secretary's Minority AIDS Initiative (MAI) Fund.

Selection Process

In identifying sites to participate in this project HealthHIV will solicit input from the other AETCs, HRSA/HAB and other federal collaborators including Bureau of Primary Health Care, state Primary Care Associations, Ryan White Part A and B grantees and HealthHIV's partners and collaborators.
