About the Project
Led by HealthHIV (formerly CAEAR Foundation) the Supporting Networks of HIV Care by Enhancing Primary Medical Care project (SNHC by EPMC) is a national technical assistance and capacity building project.
The overall goal of the project is to develop, improve, and expand comprehensive HIV primary medical care and treatment service delivery to racial/ethnic minority communities severely impacted by HIV/AIDS.
To achieve this goal, the project seeks to provide eligible primary medical care sites with services free-of-charge using the following three methods:
- Technical Assistance (TA),
- Intensive Capacity Building (ICB), and
- Web-based Training and a National Champion's Meeting
SNHC by EPMC is funded by the Health Resources and Services Administration (HRSA)’s HIV/AIDS Bureau, through the Minority AIDS Initiative.
The project is being extensively evaluated to assess its impact on the development, improvement, and expansion of high-quality HIV primary medical care for people of color.

August 7-9, 2010 HealthHIV will host SYNChronicity 2010: A National Meeting Connecting HIV & Health Networks in Washington, DC. Do not want to miss this two day event offering HIV care providers the opportunity to interact with those in primary care and coordinate efforts developing HIV medical, or health, homes. The mission of SYNC2010 is to connect SNHC by EPMC organizations and primary care providers to national HIV thought leaders, new technologies, and resources to expand their HIV practice.
SYNC2010 is a first of its kind event, bringing together SNHC by EPMC participants, HIV service providers, primary care providers, national healthcare leaders, individuals, and organizations from across the HIV community interested in integrating HIV services into the broader health care environment.

NEW modules of the SNHC by EPMC project’s Web-based Training Series for Administrators, now available.
Technical Assistance (Application's Are No Longer Being Accepted)
Technical assistance is provided at no cost in a wide range of areas to help your organization better serve clients living with HIV.
Examples of TA content areas include:
- Clinical Management of HIV Illness,
- Board Development,
- Strategic Planning,
- Medical Case Management, and
- HIV Treatment Adherence.
