How to Apply
Applying for SNHC by EPMC services is done electronically.
Organizations must:
- Meet eligibility criteria,
- Complete a Request for Service (RFS), and
- Complete specific technical assistance, training assessments and registration materials.
All the information entered into the system is kept confidential. This is a federally funded project and is required to report on its progress and on the organizations served. In these cases, all data reported will be reported in aggregate and free of any identifying information.
